Medication During School Hours
The school nurses can administer medication to students during school hours if the procedure listed below is followed:
The parent must submit a written request to the school nurse that is accompanied by a written request from the family physician in which he/she indicates the frequency and dosage of a prescribed medication. The medication should be brought to school by the parent in the original container.
Communicable Diseases
Every effort is made to keep communicable diseases out of the school to protect both your child and others. Parents are asked to keep children at home when any of the following symptoms appear:
1. Discharge from the nose.
2. Coughing.
3. Inflamed throat, or sore throat.
4. Inflammation or discharge from eyes or ears.
5. Headache, if persistent or severe.
6. Vomiting or diarrhea, within 12 hours.
7. Chills.
8. Fever, within 12 hours.
9. Body rash.
10. Lice or their eggs (nits).
When a child becomes ill in school, the parents will be called. They are responsible for seeing that an ill or injured child gets safely home or to a physician. Our school does not allow children with lice or nits (their eggs) to attend school.
Absence from School
You will want to send your child to school regularly, but please, DO NOT SEND YOUR CHILD TO SCHOOL IF HE/SHE IS ILL. When your child is absent from school, please send a written excuse to the office when he/she returns. If your child is out for more than two days, please call the office to let the school know the reason for the absence. Please note, that the district has an attendance policy. If your child is frequently absent we will begin to track your child's attendance to insure consistent attendance at school.
On the day of your child’s return from an absence, it is required that he/she bring a written excuse to the attendance office stating:
1. Date(s) of absence.
2. Cause of absence.
3. Type of illness (if absence is due to an illness).
4. Parent or guardian’s signature.
Family Matters
At times, families have confidential matters that they choose to share with the school because they feel it will benefit their child. Sometimes they believe the school needs to know about a particular event in a child’s life, (the loss of a relative, the death of a pet, a new person in the home) because it may affect his/her behavior or performance in school. Sometimes families experience changes in their family structure they want the school to be aware of so that the school knows whom to contact. Occasionally, a family may have safety concerns or custody issues the school needs to know about.
If you have information you believe someone at school should know, please call the Elementary Principal, Mrs. Paula Thoma, or School Nurse Peg McConnell. We will do our best to assure your privacy. We appreciate your keeping us informed so that we can work together.
Emergency Contact
There is nothing more frightening or frustrating than being unable to reach a parent when a child is sick or injured.
The emergency information you fill out on the blue data sheet in September is our way of knowing how to reach you or persons you select as contacts in the event of an emergency. This information is very important.
PLEASE KEEP YOUR INFORMATION UP TO DATE. If you have a change in address, phone, job location, contact persons, etc., please send a note to the school office or call us so that we will have accurate records. Thank you for your help.
IMMUNIZATION REQUIREMENTS
1. Proof of Immunization
New York State requires that Kindergarten students have documented evidence of the following immunizations:
· Polio – three doses
· DPT (Triple Vaccine) – three doses
· MMR (Measles – Mumps – Rubella) – two doses
· Hepatitis B – three doses

Area Health Departments are offering Immunization Clinics. Please call for further information. Tompkins County – 274-6616; Cortland County – 753-5203. If you do not provide us with the immunization within 14 school days after your child enters, the state mandates that we exclude your child from school until these requirements have been completed.
2. Proof of Physical Examination
NYS also requires a physical examination of all new students. Your own doctor may do physical exams, or you may choose to have it done by the school doctor. School exams will be done in early fall. If you make plans to have an exam by your doctor, be sure to schedule it on or after June 1st. We would appreciate being notified of your child’s scheduled appointment. 3. School Health Information Form
Please fill out the School Health Information Form completely and bring it along with documentary evidence of your child’s immunizations when you bring your child to Kindergarten Visitation.
In the event of illness, we ask that you keep your child home until he/she is symptom free. A written excuse should accompany them upon their return to school. In the event that medication will need to be administered during the school day, please keep in mind that it can only be given when accompanied by a written note from your family doctor and brought to school in the original container.
